Membership Terms

Membership is a separate recurring subscription of $49.99 per month. Laboratory tests are purchased separately. Unlimited home draws apply only to eligible collection visits and are subject to service area, appointment availability, operating hours, and Membership Terms.

Subscription and billing

Membership is a separate recurring subscription of $49.99 per month, billed monthly until cancelled.
Laboratory tests are purchased separately and are not included in the monthly membership fee.
Membership benefits apply only while the membership is active and paid.

Eligible home draws

Unlimited home draws apply only to eligible collection visits.
Eligible visits are subject to service area, appointment availability, operating hours, and these Membership Terms.
Visits outside the published service area, outside operating hours, or requiring special handling may not be eligible.
The $0 home-draw fee applies to the collection visit only.

Member test discount

The 20% discount applies to eligible laboratory tests purchased by the member while the membership is active.
Certain tests, bundles, and third-party charges may be excluded from the discount.
Discounts cannot be combined with other offers unless stated at checkout.

Cancellation

Membership can be managed and cancelled online at any time from the member account area.
Cancellation stops future monthly charges; benefits remain available through the end of the paid billing period.
