Eligible standard home draws
- The $0 fee applies to standard eligible at-home collection appointments only.
- Eligible visits cover one named adult member, within approved service areas, during standard weekday appointment windows and normal operating hours.
- One standard collection visit connected to an eligible laboratory order is included per appointment.
- Same-day priority, after-hours, weekend, holiday, out-of-area, therapeutic, timed multi-hour, glucose-tolerance, specialty-handling, and long-observation collections are excluded unless separately approved.
- Repeat visits caused by inaccurate information or unmet preparation instructions, no-shows, and late cancellations are excluded.
- Membership benefits apply only to the named member and cannot be transferred or shared.
Member test discount
- The 20% discount applies only to eligible laboratory-test prices while the membership is active.
- Outside laboratory, shipping, courier, specialty-processing, consultation, telehealth, medication, and tax charges are excluded.
- Testing that requires a valid physician order, licensed-provider review, or other authorization must still satisfy those requirements.
Rescheduling, pauses and cancellation
- Eligible appointments may be rescheduled free of charge when requested at least 24 hours in advance.
- One optional membership pause per 12-month period may be available if operationally approved.
- Membership can be managed and cancelled online at any time from the member account area.
- Cancellation stops the next recurring charge; benefits remain available through the end of the paid period, and a confirmation is emailed.
