A result within the reference range does not rule out illness, and a result outside the reference range does not necessarily mean that a medical condition is present. • I understand that normal laboratory results do not rule out a medical condition and abnormal laboratory results do not independently establish a diagnosis.
• I understand that laboratory testing may produce unexpected, inconclusive, false-positive, or false-negative results and that additional testing or medical evaluation may be necessary.
• I understand that Signature Draws is not an emergency service. I will call 911 or seek immediate medical care for severe, rapidly worsening, or emergency symptoms.
• I confirm that the information I provided is accurate to the best of my knowledge and that I have reviewed the selected tests, included markers, limitations, preparation instructions, specimen requirements, and applicable charges.
• I understand that the decision to purchase self-selected testing is my decision. I accept responsibility for selecting the tests I order and for obtaining appropriate professional medical advice and follow-up.
• I understand that I should not start, stop, or change any medication, supplement, treatment, or healthcare plan based solely on this questionnaire or laboratory results.

I confirm that I am at least 18 years old and am ordering for myself, or that I am legally authorized to order and consent on behalf of the person being tested.

If testing is for a minor, a separate parent or legal-guardian consent process applies. The adult acknowledgment above is not used on its own for minors.

I understand that I am voluntarily selecting and purchasing the laboratory testing listed in my order. Signature Draws has not represented that any selected test will diagnose, rule out, prevent, monitor, or explain any particular medical condition or symptom. I accept responsibility for my self-selected testing decisions, for providing accurate information, for following collection and preparation instructions, and for seeking appropriate medical advice regarding my symptoms and results. To the fullest extent permitted by applicable law, I accept the risks associated with relying on self-selected laboratory testing without first consulting a licensed healthcare professional. Nothing in this acknowledgment waives any right or responsibility that cannot legally be waived, nor does it release any party from liability for conduct that cannot lawfully be limited or excluded.

By signing below, I confirm that I have read, understood, and agree to the Important Medical Acknowledgment.

The sign-off collects your full legal name, the date, a typed-name electronic signature, your agreement to this acknowledgment, and your agreement to the Terms of Service and Privacy Notice. Each box is unchecked by default and requires your action.

This acknowledgment does not replace any separate informed consent, laboratory authorization, physician-order, telehealth, privacy, or regulatory documentation required by law or by the performing laboratory.
